Does Creatine Really Cause Acne? We’re Setting the Record Straight

It’s one of the most persistent rumors in the gym: start taking creatine, and your skin pays for it. Scroll through enough fitness forums and you’ll find plenty of people swearing their breakouts started the same week they started supplementing. So is there anything to it, or is creatine just an easy scapegoat for something else entirely? Let’s look at what’s actually going on.

Where the Acne Rumor Actually Comes From

The theory traces back to a small, often-cited study linking creatine supplementation to a rise in a hormone called DHT, which is associated with oilier skin and, in some people, breakouts. It’s a real finding worth acknowledging — but it’s one study, with a small sample, and it hasn’t been consistently replicated. The broader body of research on creatine, which is some of the most well-studied supplementation in sports science, hasn’t found acne to be a common or predictable side effect.

Some lifters try cycling on and off creatine specifically to “give their skin a break,” assuming time off will reset things — but there’s little evidence that stopping and starting changes your skin’s behavior one way or the other.

For most people, breakouts that show up around the time they start creatine have a much more mundane explanation: a new training block, a new pre-workout, more sweat sitting on the skin after longer sessions, or just normal hormonal fluctuation that would’ve happened anyway. Correlation isn’t causation, and creatine timing lining up with a breakout doesn’t mean it caused it.

It’s also worth noting this isn’t unique to supplements — creatine from food sources affects hormones the same way a supplement does, since your body doesn’t distinguish between the two sources.

Skin concerns tend to surface around the same time as other early adjustments, like the water-weight shift many people notice in their first couple weeks — both get blamed on the supplement when they’re really just part of your body adapting to a new routine.

If skin is still a genuine concern for you, the honest move is to track it — note if breakouts actually track with dosing over several weeks rather than assuming based on one bad week.

If you’re still cautious, taking it post-workout after a shower, rather than right before training, can rule out sweat-related irritation as a factor.
